Sarah Jane has worked within Social Services for 14 years and qualified as an Occupational Therapist in 2009. She has experience working in long term patch teams specialising in the provision of major adaptations. In 2018 Sarah Jane began her career in Cheshire West and Chester where she was given the opportunity to lead their Single Handed Care project. Sarah Jane is now the Practice Manager of a Home Assessment Team embracing the discharge to assess model working with the wider Reablement service.
