Michelle is a Service Manager for Adults’ Health and Care Reablement Service in Hampshire. Michelle has worked in Social Care for almost 20 years and is a qualified Occupational Therapist by background. The Reablement service formed in 2017 and comprises of Occupational Therapists, Sensory Officers and CQC registered Reablement care delivery staff. Michelle works in partnership with both health and social care colleagues to support pathways out of hospital as well as prevention of admission. Michelle is passionate about service development and innovation, seeking ways to continually improve services and outcomes.
