Clare Smith is a Senior Clinical Practitioner within Home Group's Central Clinical Team, providing specialist clinical expertise across national complex care services. She supports individuals with learning disabilities, autism, mental health conditions, and complex care needs, drawing on extensive experience in occupational therapy assessment, Positive Behaviour Support, risk management, and environmental adaptations. Recognised for her compassionate and collaborative approach, Clare works closely with multidisciplinary teams to develop evidence-based interventions and bespoke support plans. Her practice focuses on promoting independence, developing skills, enhancing meaningful participation, and improving quality of life, ensuring individuals are empowered to achieve their personal goals and aspirations. As a PROACT-SCIPr-UK Instructor, Clare delivers specialist training in proactive and person-centred approaches to supporting individuals who present with behavours that challenge. She equips teams with practical and therapeutic strategies for prevention and de-escalation whilst using the least-restrictive practices.
