The Home Team delivers person centred care with the promotion of independence at the core of service delivery. We welcome applications from enthusiastic registered Occupational Therapists with a keen interest in working within an adult community health setting with a focus on utilising professional skills to deliver rehabilitation and enablement.

The focus for this post is provision of Occupational Therapy assessment and intervention to help maximise function and independence, focussing predominantly on short term rehabilitation, proactive and preventative interventions. The post holder is expected to expertly manage a caseload to provide a full service of assessment, appropriate management, onward referral and discharge to a range of patients referred to the service. The post sits within a close team of multi-professionals who are working together to provide excellent and timely service within the local community.
