Life after discharge: case managing complex neurological rehabilitation in the community
Discharge from hospital is only the beginning of the journey for many people living with complex neurological conditions. Coordinating rehabilitation in the community can involve multiple professionals, services and funding arrangements, with the case manager playing a pivotal role in bringing these elements together.
This session will explore the challenges and opportunities of coordinating complex neurological rehabilitation beyond hospital discharge, highlighting the role of case management in facilitating multidisciplinary collaboration, managing risk, advocating for the individual and ensuring rehabilitation remains focused on meaningful, person-centred outcomes. The session will consider how effective case management can bridge gaps between services, support families and help people achieve their goals, maximise independence and improve quality of life.
