Self Assessment for Frailty: a pilot service.

I have teamed up with Lancaster University to start a new service offering self assessments for Frailty and we plan to go live this Autumn. This will include a 5 year research study to evaluate the impact of early assessments and intervention. We have a UKRi grant. This is a great opportunity to share our plan with a key Professional Group: Occupational Therapists
- I will describe the self assessment, digital version of COMFI-25 and how this will direct a combination of home visit assessments and a Virtual MDT.
